I've gone back to eating hard boiled eggs and yogurt and have been able to cut the insulin back to 30 units in the morning and taking my Avandia twice a day and the Glucophage once a day. I'm still working up the energy to walk and ride bike but with the screwy weather we're having right now I wouldn't want to walk to far anyway. My fasting BS for the past 3 weeks has been between 120 and 150 and for me thats good. Thats without the night time insulin and no pill before bed. Well, time to get ready for the evening out. I will have my fish combo, it's my weekly treat, BS is still in the range on Thursday morning.

In decreasing order of probability they are insufficient insulin, dawn phenomenon, and Somogyi effect (aka rebound). Insufficient or waning insulin is simple. If the effective duration of intermediate or long acting insulin ends sometime during the night, the relative level of circulating insulin will be too low, and your blood sugars will rise. Dawn phenomenon refers to increased glucose production and insulin resistance brought on by the release of counterregulatory hormones in the early morning hours near waking. It happens in normal people as well as in diabetics; in nondiabetics it shows up as measurably increased insulin secretion around dawn. Dawn phenomenon is variable in strength both within the population and over time in individuals. It can show up as either high fasting glucose levels or an increased insulin requirement to cover breakfast compared to equivalent meals at other times of day. Somogyi effect refers to a rebound in bg after nocturnal hypoglycemia which occurs during sleep with the patient not experiencing any symptoms. The hypoglycemia triggers the release of counterregulatory hormones. Somgoyi effect appears to be less prevalent than previously thought. While it does occur, some episodes of hyperglycemia following hypoglycemia are actually waning insulin levels following an insulin peak with medium acting insulin. This can be difficult to sort out. The best way to sort it out is to test every couple of hours from bedtime to morning. If your bg rises all, or much of the night, it is a lack of circulating insulin.If it is stable all night, but rises sharply sometime before you wake in the morning, it is dawn phenomenon. If your bg declines to the point of a hypoglycemic reaction, it is *possibly* Somogyi effect.You may have to test on several nights to nail the problem. Once you have figured out the problem you and your doctor can discuss changes in your insulin regimen to correct it. The answer depends critically on your particular circumstances. *Mayer Davidson, in _Diabetes Mellitus: Diagnosis and Treatment_ (p 252 in the 3rd edition) says that Somogyi effect rarely causes fasting hyperglycemia,and cites studies.

The error can be discovered by testing blood glucose every two hours or so during the day and night to try to find out whether the insulin dose actually is causing hypoglycemia. Another clue would be that the response to increasing the insulin dose is higher rather than lower blood glucose levels. The dosage may have to be lowered or spread over several injections throughout the day to avoid the rebounding effect. Overtreating a reaction can also cause rebounding. Because of the resulting high blood glucose, the person doesn't eat all his or her food and takes extra short-acting insulin. Never increase insulin or decrease the amount of food on the basis of one or two high blood glucose tests unless urine ketones are present. Always wait to see if a pattern develops over several days. Then if you're not sure how to adjust insulin, or if blood glucose is not controlled by your adjustments, discuss the situation with your health care team. From Live well, with diabetes. JOHN.

We had more rain and heavy winds today but nothing like Friday was. No damage done today. They are predicting more rain and heavy winds for the rest of the week. Most of the day is really nice though. We can expect a couple of more days in the 80's. I love the warm weather. Now to get up the energy to start walking and riding bike. Once I get started I'll be fine, it's getting started thats the hard part. Everyone have a wonderful evening

CB...I was at a lecture last night about a book you might find interesting. He talks a lot about (at least in the talk) preformance athletics and nutrition. He's been involved with some pretty high profile athletes. Its clled "Fat Won't Make You Fat" "Not all by itself". by L. Lee Coyne, Ph.D. The title he says now may have been a boo boo but he was trying to make a point. The research he uses supports very strongly a 40-30-30 program. Carbs being the 40.
